Video-Assisted Thoracoscopic Surgery for the Plication of High-Level Thoracic Congenital Diaphragmatic Eventration

Abstract

Congenital diaphragmatic eventration (CDE) results in an abnormal elevation of the diaphragm because of maldevelopment of its muscular portion, allowing the abdominal contents to herniate into the chest. It may be an incidental finding on chest radiography as this condition can be relatively asymptomatic. Furthermore, CDE is occasionally difficult to distinguish from congenital diaphragmatic hernia with a sac as both conditions may present with similar lung manifestations. A high index of suspicion of an unusually elevated diaphragm with immediate bedside ultrasonography confirmation may lead to rapid diagnosis of CDE and early surgical referral of affected neonates. In pediatric patients, video-assisted thoracoscopic plication offers a rapid improvement of the respiratory status and recovery than that observed through thoracotomy.
